Rapamycin (sirolimus) is a macrolide compound originally isolated from the bacterium Streptomyces hygroscopicus found in soil samples from Easter Island (Rapa Nui) in 1972. It is FDA-approved as an immunosuppressant to prevent organ transplant rejection (brand name Rapamune, approved 1999) and is also used in drug-eluting coronary stents. It works by inhibiting the mechanistic target of rapamycin (mTOR), a kinase that serves as a central regulator of cell growth, metabolism, and aging.,Rapamycin has become one of the most studied compounds in longevity research. It is the only drug consistently shown to extend lifespan across multiple species, including yeast, worms, flies, and mice. The National Institute on Aging's Interventions Testing Program confirmed that rapamycin extends mouse lifespan even when started late in life. This has generated enormous interest in low-dose, intermittent rapamycin protocols for human healthspan extension.,At immunosuppressive doses (daily use), rapamycin has well-documented side effects including immunosuppression, hyperlipidemia, and impaired wound healing. However, the longevity community uses intermittent low-dose protocols (typically weekly) which may achieve partial mTOR inhibition with a different, potentially more favorable side effect profile. This distinction between chronic high-dose and intermittent low-dose rapamycin is critical but not yet fully characterized in human clinical trials.
