Gonadorelin is a synthetic decapeptide identical to endogenous gonadotropin-releasing hormone (GnRH) produced by the hypothalamus. It binds to GnRH receptors on the anterior pituitary gland, stimulating the synthesis and release of luteinizing hormone (LH) and follicle-stimulating hormone (FSH). These gonadotropins are essential for reproductive function, stimulating testosterone production in males and ovulation in females.,Gonadorelin has been FDA-approved since the 1980s for diagnostic evaluation of pituitary gonadotroph function and for the treatment of primary hypothalamic amenorrhea. In clinical practice, pulsatile GnRH administration via pump can restore normal pulsatile gonadotropin secretion and fertility. It is also used off-label in male hormone optimization protocols to maintain endogenous testosterone production and fertility during testosterone replacement therapy.,Due to its extremely short half-life (2 to 4 minutes), gonadorelin must be administered in pulsatile fashion or at frequent intervals to mimic the body's natural secretion pattern. Continuous or high-dose administration paradoxically suppresses LH and FSH through receptor downregulation, which is the basis for GnRH agonist drugs used in prostate cancer and endometriosis.
