Hesperidin is a flavanone glycoside abundantly found in citrus fruits, especially the white pith and inner peel of oranges, lemons, and tangerines. It's the most-consumed citrus flavonoid in the typical Western diet (50-200mg/day from juice and citrus consumption). Hesperidin has been used since the 1930s in European pharmaceuticals for chronic venous insufficiency and hemorrhoids (typically as Diosmin/Hesperidin combinations like Daflon), and modern research has expanded its applications to cardiovascular health, anti-inflammatory effects, and metabolic support.

Take with meals to improve absorption (Hesperidin is fat-soluble). For venous symptoms or hemorrhoidal flares, look for Diosmin/Hesperidin combination products (Daflon-style 500mg total). WHAT WAS ACTUALLY MEASURED: flow-mediated dilation (10.26% vs 7.78% on placebo) and inflammatory markers - hsCRP, serum amyloid A, soluble E-selectin. It did NOT measure body composition, testosterone or performance, and the authors call it exploratory. The dose is also form-specific: pure hesperidin has low bioavailability, and the trials used defined preparations (2S-hesperidin, glucosyl-hesperidin), so a generic extract at the same milligrams is not equivalent.

How it works

Hesperidin's primary vascular mechanisms include strengthening capillary walls (reducing leakiness via stabilization of endothelial junctions), enhancing venous tone (mild norepinephrine-potentiating effect on venous smooth muscle), reducing inflammation in venous valves (anti-inflammatory effects on perivenous tissue), and supporting lymphatic drainage. The combined effects underlie the well-established efficacy in chronic venous insufficiency and hemorrhoidal flares.

Cardiovascular effects involve eNOS upregulation (improving endothelial NO production for vascular relaxation), modest reduction of vascular oxidative stress, and BP-lowering effect possibly via AT1 receptor antagonism and ACE inhibition (mild). Hesperidin also reduces platelet aggregation and may have modest antithrombotic effects, contributing to cardiovascular protection.

Anti-inflammatory mechanisms include NF-kB pathway inhibition (reducing TNF-alpha, IL-6, COX-2 expression), modulation of nitric oxide synthase activity, and direct antioxidant effects via the flavanone structure. Metabolic effects involve modest improvements in insulin sensitivity (mechanism via GLUT4 upregulation, AMPK activation), hepatic lipid metabolism (reduced hepatic triglyceride accumulation in animal models), and adipose tissue function. The Diosmin form is more bioavailable and is the form used in most pharmaceutical preparations.
