Take with the largest meal (especially containing fat) for fat-soluble vitamin absorption (A, D, E, K). Iron-containing multis on empty stomach when possible (better absorption) but with food if causes nausea. Split-dose multis (twice daily) provide more consistent blood levels for water-soluble vitamins.

How it works

Multivitamins provide essential vitamins and minerals as cofactors for thousands of enzymatic reactions throughout the body. Many enzymes have absolute requirements for specific cofactors - missing one means the reaction doesn't proceed efficiently or at all.

B-vitamins (thiamine, riboflavin, niacin, pantothenic acid, B6/P5P, biotin, folate, B12) are essential for energy metabolism, methylation, neurotransmitter synthesis, and DNA synthesis/repair. Methylation in particular drives detoxification, gene expression, and homocysteine clearance - inadequate folate/B12/B6 raises cardiovascular risk.

Antioxidant vitamins (C, E, beta-carotene, selenium): support endogenous antioxidant defense and neutralize ROS. Note: high-dose antioxidant supplementation has shown mixed results in trials (some negative for smokers/cancer patients) - moderate doses in multivitamins are well-tolerated.

Minerals (zinc, copper, magnesium, manganese, selenium, chromium, molybdenum, iodine): cofactors for hundreds of enzymes. Zinc:copper ratio matters (10-15:1 prevents copper deficiency). Selenium supports thyroid hormone activation and glutathione peroxidase. Iodine supports thyroid hormone synthesis (often inadequate in non-iodized salt diets).

Vitamin D3 + K2: provides baseline cardiovascular and bone health support (covered in detail in the dedicated D3+K2 entry).

Special considerations: iron should generally NOT be in male/post-menopausal multivitamins (excess iron is pro-oxidant and damages tissues). Beta-carotene at very high doses may increase lung cancer risk in smokers (separate concern from typical multi doses).

Methylation support: methylated B vitamins (methylfolate, methylcobalamin, P5P) bypass MTHFR and other genetic variants that reduce conversion efficiency from synthetic forms (folic acid → methylfolate). ~25-50% of population has reduced methylation efficiency genetically - methylated forms benefit them.
