Acetyl-L-Carnitine (ALCAR) is the acetylated form of L-carnitine, a quaternary ammonium compound essential for fatty acid oxidation and cellular energy production. Endogenously synthesized from lysine and methionine, ALCAR readily crosses the blood-brain barrier (unlike standard L-carnitine which has limited CNS penetration), making it the preferred carnitine form for neurological applications. ALCAR is found in trace amounts in foods including red meat, dairy, and to lesser extent fish and poultry.

How it works

ALCAR's primary mechanism is delivery of bioavailable carnitine into cells (especially neurons) where it shuttles long-chain fatty acids across the inner mitochondrial membrane for beta-oxidation - a critical step in mitochondrial ATP production. The acetyl group on ALCAR provides additional acetyl-CoA equivalents that enter the citric acid cycle, supporting mitochondrial energy output. ALCAR also crosses the blood-brain barrier readily (unlike L-carnitine), supporting neuronal mitochondrial function.

ALCAR has direct cholinergic effects via acetyl group donation for acetylcholine synthesis (cholinergic component of cognitive support) and supports nerve growth factor (NGF) signaling - particularly relevant for the neuropathy applications. The compound stimulates protein kinase C signaling in neurons and modulates expression of genes involved in cellular stress response and longevity (including SIRT1/SIRT3 pathway interactions).

Additional mechanisms include modulation of monoamine neurotransmitters (dopamine, serotonin, norepinephrine - underlying antidepressant effects), reduction of oxidative stress in nervous tissue, support for sperm membrane fluidity and motility (fertility application), and modest GLUT4 upregulation supporting glucose handling. ALCAR also has chelating effects, binding excess copper - relevant for some neurodegenerative applications.

Dose boundary: trials tested acetyl-L-carnitine at 500 mg/day or 1,000 mg/day total (each given in divided doses three times daily), not the card's 500 mg twice-daily schedule. The 1,000 mg/day arm's total daily amount matches the card's regimen (500 mg b.i.d. = 1,000 mg/day) — only the administration split (t.i.d. vs. b.i.d.) differs.

We evaluated frozen databases from two 52-week randomized placebo-controlled clinical diabetic neuropathy trials testing two doses of acetyl-L-carnitine (ALC): 500 and 1,000 mg/day t.i.d.

Human effect: ALCAR improved pain, nerve-fiber regeneration, and vibration perception in diabetic neuropathy trials.

These studies demonstrate that ALC treatment is efficacious in alleviating symptoms, particularly pain, and improves nerve fiber regeneration and vibration perception in patients with established diabetic neuropathy.

Safety: randomized-trial evidence in a review found no significant difference in adverse reactions versus placebo and characterized ALCAR as generally well tolerated.

In terms of adverse reactions, no significant differences were observed between treatment and placebo groups, implying that ALC is generally well-tolerated.

Exact dose/frequency: 500 mg oral twice daily. The checked abstract used 500 or 1,000 mg three times daily; that does not verify twice-daily dosing.
