L-Carnitine (levocarnitine) is a naturally occurring amino acid derivative synthesized from lysine and methionine, primarily in the liver and kidneys. Its essential biological role is transporting long-chain fatty acids across the inner mitochondrial membrane via the carnitine shuttle system (carnitine palmitoyltransferase I and II), enabling beta-oxidation for energy production. Without adequate carnitine, cells cannot efficiently oxidize fatty acids for ATP generation.

How it works

L-Carnitine serves as the essential carrier molecule for long-chain fatty acyl-CoA transport across the inner mitochondrial membrane. It binds to fatty acyl groups via carnitine palmitoyltransferase I (CPT-I), crosses the membrane, and releases them via CPT-II for beta-oxidation. This shuttle is the rate-limiting step for long-chain fatty acid oxidation.

Beyond fatty acid transport, L-carnitine functions as a buffer for the acyl-CoA/CoA ratio inside mitochondria, accepting excess acyl groups to maintain free CoA availability. This protects mitochondrial function during metabolic stress and supports the citric acid cycle and pyruvate dehydrogenase activity.

L-Carnitine also modulates gene expression related to mitochondrial biogenesis and antioxidant defense, and has been shown to improve insulin sensitivity and glucose disposal in some clinical studies, possibly through enhanced fatty acid oxidation and reduced intracellular lipid accumulation.
