L-Carnitine (levocarnitine) is a naturally occurring amino acid derivative synthesized from lysine and methionine, primarily in the liver and kidneys. Its essential biological role is transporting long-chain fatty acids across the inner mitochondrial membrane via the carnitine shuttle system (carnitine palmitoyltransferase I and II), enabling beta-oxidation for energy production. Without adequate carnitine, cells cannot efficiently oxidize fatty acids for ATP generation.,Injectable levocarnitine (Carnitor IV) is FDA-approved for the treatment and prevention of carnitine deficiency in patients undergoing hemodialysis. Primary carnitine deficiency is a genetic condition causing severe muscle weakness, cardiomyopathy, and hypoglycemia. Secondary deficiency can occur in dialysis patients (carnitine is dialyzable), individuals on valproic acid therapy, and those with certain organic acidemias. Injectable administration bypasses variable oral bioavailability (14-18% for oral carnitine).,Beyond its approved indications, injectable L-carnitine is widely used in wellness, weight management, and athletic performance contexts. While the biochemical rationale for fat metabolism support is sound, clinical evidence for weight loss or performance enhancement in carnitine-replete individuals is modest. Injectable delivery achieves substantially higher plasma levels than oral supplementation, which has led to its popularity in optimization protocols.

L-Carnitine serves as the essential carrier molecule for long-chain fatty acyl-CoA transport across the inner mitochondrial membrane. It binds to fatty acyl groups via carnitine palmitoyltransferase I (CPT-I), crosses the membrane, and releases them via CPT-II for beta-oxidation. This shuttle is the rate-limiting step for long-chain fatty acid oxidation.,Beyond fatty acid transport, L-carnitine functions as a buffer for the acyl-CoA/CoA ratio inside mitochondria, accepting excess acyl groups to maintain free CoA availability. This protects mitochondrial function during metabolic stress and supports the citric acid cycle and pyruvate dehydrogenase activity.,L-Carnitine also modulates gene expression related to mitochondrial biogenesis and antioxidant defense, and has been shown to improve insulin sensitivity and glucose disposal in some clinical studies, possibly through enhanced fatty acid oxidation and reduced intracellular lipid accumulation.
