Iodine is an essential trace mineral required for thyroid hormone synthesis - thyroxine (T4) contains 4 iodine atoms and triiodothyronine (T3) contains 3. Without adequate iodine, the thyroid cannot produce sufficient hormone, leading to hypothyroidism, goiter (compensatory thyroid enlargement), and severe developmental consequences in iodine-deficient pregnancies (cretinism). Iodine is found primarily in seaweed (kelp, nori, dulse), seafood, dairy products (in some countries through iodized cattle feed and cleaning agents), eggs, and iodized salt.

How it works

Iodine's primary mechanism is incorporation into thyroglobulin protein at the thyroid follicle apical membrane, where TPO (thyroid peroxidase) catalyzes oxidation and coupling of iodinated tyrosine residues to produce T4 and T3. The thyroid actively concentrates iodine via the Na+/I- symporter (NIS), achieving 20-40x serum levels. After production, T4 and T3 are stored in colloid and released as needed for systemic effects.

Beyond thyroid hormone synthesis, iodine has functions in extrathyroidal tissues including breast, ovary, prostate, stomach, and salivary glands. The breast tissue connection is particularly notable - breast tissue concentrates iodine, and iodine deficiency may contribute to fibrocystic breast disease. Animal studies show iodine has direct apoptotic effects in some breast cancer cell lines (mechanism via 6-iodolactone formation and PPAR-gamma activation), supporting the epidemiological correlation between dietary iodine and lower breast cancer rates.

At pharmacological doses, iodine has additional effects including antimicrobial activity (basis for povidone-iodine wound disinfection), bromide and fluoride displacement (some functional medicine practitioners use high-dose iodine to displace toxic halides accumulated from environmental exposure), and modulation of inflammatory pathways. The Wolff-Chaikoff effect describes the temporary reduction in thyroid hormone synthesis when intracellular iodine reaches very high concentrations - an autoregulatory mechanism that protects against iodine excess but can cause transient hypothyroidism in sensitive individuals.
