Pentosan polysulfate sodium (PPS, brand name Elmiron) is a semi-synthetic heparinoid derived from beech wood hemicellulose. It is the only FDA-approved oral treatment for the pain and discomfort of interstitial cystitis (IC), also known as bladder pain syndrome. It was approved in 1996 and works by replenishing the glycosaminoglycan (GAG) layer of the bladder epithelium, which is thought to be deficient in IC patients.
Starting dose. 100 mg — Oral (Elmiron)
Take with water at least 1 hour before or 2 hours after meals. NOTE: the 2-3mg/kg SubQ injection widely quoted for osteoarthritis is the VETERINARY product (Cartrophen, Zycosan) and is not an approved human route. Pentosan carries a documented risk of pigmentary maculopathy with long-term use - baseline and periodic retinal exams are advised.
