Pentosan polysulfate sodium (PPS, brand name Elmiron) is a semi-synthetic heparinoid derived from beech wood hemicellulose. It is the only FDA-approved oral treatment for the pain and discomfort of interstitial cystitis (IC), also known as bladder pain syndrome. It was approved in 1996 and works by replenishing the glycosaminoglycan (GAG) layer of the bladder epithelium, which is thought to be deficient in IC patients.,Beyond its bladder indication, PPS has attracted significant research interest for its anti-inflammatory, anti-thrombotic, and tissue-protective properties. It has been investigated for osteoarthritis, prion diseases, and more recently for its potential regenerative effects on joints and soft tissue. Injectable forms of PPS are used extensively in veterinary medicine for osteoarthritis in dogs and horses, and human injectable protocols have gained interest in integrative and longevity medicine.,A notable safety concern emerged in 2018 when reports linked long-term oral PPS use to a unique pigmentary maculopathy affecting the retina. The FDA added a warning to the Elmiron label in 2020 regarding this risk, primarily associated with prolonged oral use at standard doses. The risk profile for lower-dose injectable protocols is less well characterized.
